ELIGIBILITY FOR HOSPICE SERVICES
Anyone who has a life-limiting illness and a life expectancy of six months or less is certified by their attending physician and hospice physician. The patient or the representative chooses comfort care instead of curative or aggressive treatment, which is no longer effective or desired.
WHO PROVIDES CARE?
Care is delivered by a team of professionals who address the physical, emotional, and spiritual needs of the patients and their loved ones. The team consists of the patient’s attending physician, hospice medical director, registered nurse, hospice aids, social worker, spiritual counselors, therapists, and trained volunteers.
ADMISSION PROCESS
Admitting a patient to the hospice is a simple process: just call the hospice, and a Licensed Nurse/Physician will visit the patient. Information about the hospice will be presented, and if the patient or the family elects hospice care, the nurse will arrange for the start of services.
